Suicide is the 10th leading cause of death overall in the United States and the second leading cause of death in individuals aged 10-34 years, though it remains dramatically underestimated in clinical practice. The lifetime prevalence of suicide attempts is approximately 4-5% in the general population, with males completing suicide 3-4 times more frequently than females despite females attempting more frequently (the "gender paradox"). Every clinician across all specialties must develop competence in suicide risk assessment because suicidality frequently presents in medical settings, psychiatric patients are overrepresented among those who die by suicide, and systematic assessment significantly reduces risk. The ability to perform a thorough suicide risk assessment, stratify risk level, implement safety planning, and coordinate care is essential board knowledge and fundamental to safe medical practice.

The biological basis of suicidal behavior involves dysregulation across multiple interconnected neurobiological systems, with particular emphasis on monoaminergic dysfunction, stress-response abnormalities, and structural brain changes. Suicide represents the behavioral endpoint of complex interactions between genetic predisposition, trait-like neurobiological vulnerabilities (particularly related to impulsivity and aggression), and acute precipitating stressors that interact within a framework of psychiatric illness.

  • Serotonergic dysfunction as the primary neurochemical pathway: The serotonergic system originating from raphe nuclei projects widely throughout the prefrontal cortex, anterior cingulate, amygdala, and limbic regions central to mood, impulse control, and decision-making. Reduced cerebrospinal fluid (CSF) 5-hydroxyindoleacetic acid (5-HIAA), a serotonin metabolite, has been consistently found in individuals who die by suicide, particularly those with violent methods. Suicidal behavior exists on a spectrum from ideation (passive and active) through planning and preparation to attempt and completion. Clinical presentation varies dramatically based on psychiatric diagnosis, personality traits, and concurrent stressors, but systematic assessment using validated frameworks is essential.

  • Suicidal ideation (SI) as the cardinal symptom with variable manifestations: Passive suicidal ideation involves the wish to be dead or that something would happen to cause death, without active planning or intent to act—for example, wishing not to wake up from sleep or that a car accident would happen. Passive SI is more common and generally carries lower acute risk, though it should not be dismissed as "safe." **

Suicide risk is a clinical judgment, not a test result; no laboratory study, imaging finding, or scale score can rule risk in or out. The diagnostic sequence moves from universal screening to a structured risk formulation.

Step 1 — Screening (initial "test")

  • Validated screening instruments: The Joint Commission National Patient Safety Goal 15.01.01 requires screening all patients whose primary reason for care is a behavioral health condition using a validated tool. Common instruments are the Ask Suicide-Screening Questions (ASQ), the Columbia-Suicide Severity Rating Scale (C-SSRS) screener, and item 9 of the PHQ-9 (thoughts of being better off dead or of self-harm), which is positive at any response above "not at all."
  • Screening context: The USPSTF recommends depression screening in adults, including pregnant and postpartum persons, and in adolescents; for stand-alone suicide-risk screening in asymptomatic populations the USPSTF has issued an insufficient-evidence (I) statement. A positive screen mandates evaluation, never simply repeat testing.

Step 2 — Confirmatory structured assessment (gold standard)

  • Direct clinical interview covering the ideation-to-action spectrum: passive wish to die → active ideation → method → plan, intent, preparatory acts, rehearsal, and access to means (especially firearms). The C-SSRS grades ideation 1–5 (1 = wish to be dead; 5 = active ideation with specific plan and intent) and separately codes behaviors (aborted, interrupted, actual attempt, preparatory acts).
  • Corroborating history: prior attempts and their lethality/rescuability, psychiatric diagnosis, substance use, hopelessness, insomnia, agitation, recent discharge, and protective factors.

Step 3 — Medical evaluation after an attempt

  • Toxicology and metabolic panel: serum acetaminophen level at 4 hours post-ingestion plotted on the Rumack-Matthew nomogram, salicylate level, ethanol, glucose, electrolytes, and pregnancy testing.
  • ECG: in suspected tricyclic overdose, QRS >100 ms predicts seizure and >160 ms predicts ventricular arrhythmia.

Step 4 — Risk formulation: The APA practice guideline framework stratifies acute versus chronic risk (low/moderate/high) to drive disposition, rather than relying on a numeric score. Actuarial checklists such as SAD PERSONS have poor predictive validity and should not determine disposition.

Immediate stabilization

  • Environmental safety first: continuous 1:1 observation, removal of belongings and ligature points, and search for medications or weapons. The Joint Commission requires a ligature-resistant environment for patients at high risk in psychiatric units.
  • Medical stabilization of the attempt precedes psychiatric disposition: ABCs, then antidote-directed care — N-acetylcysteine for acetaminophen, naloxone for opioids, sodium bicarbonate for tricyclic-induced QRS widening, flumazenil generally avoided (seizure risk in mixed ingestion/chronic benzodiazepine use).
  • Disposition: voluntary admission is preferred; if the patient has a mental illness and is a danger to self and refuses, involuntary hold/civil commitment under state statute is indicated. Refusal of care in this setting does not require the patient to be globally incompetent, only to meet statutory dangerousness criteria.

Core interventions for all at-risk patients (APA guideline; Zero Suicide model)

  • Safety planning: the Stanley-Brown Safety Planning Intervention — warning signs, internal coping, contacts, crisis line (988), and means restriction. "No-suicide contracts" are not evidence-based and should not be used.
  • Lethal means counseling: firearm storage off-site or locked; limiting pill quantity dispensed.

Disease-directed pharmacotherapy

  • SSRIs (e.g., sertraline) are first-line for the underlying depressive or anxiety disorder; effect on suicidality follows symptom improvement over weeks.
  • Lithium has the strongest evidence for reducing suicide in bipolar disorder and recurrent depression — but it is itself lethal in overdose, so quantity must be limited.
  • Clozapine is FDA-approved to reduce recurrent suicidal behavior in schizophrenia and schizoaffective disorder.
  • Esketamine (intranasal, with an oral antidepressant) is FDA-approved for depressive symptoms with acute suicidal ideation or behavior, given under REMS with post-dose monitoring.

Escalation

  • Electroconvulsive therapy for severe depression with imminent suicidality, catatonia, psychotic depression, or refusal to eat/drink.
  • Psychotherapies with attempt-reduction evidence: CBT for suicide prevention and dialectical behavior therapy (first-line for borderline personality disorder).

Avoid: discharge without a scheduled follow-up (contact within about a week post-discharge is standard), antidepressant monotherapy in bipolar depression, and reflexive benzodiazepine use in impulsive or intoxicated patients.

Complications of the disorder

  • Completed suicide — the terminal outcome; a prior attempt is the single strongest predictor, and risk is concentrated in the days to weeks after psychiatric hospitalization or ED discharge, when supervision drops but hopelessness persists. Emergency.
  • Repeat attempt with escalating lethality: each attempt reinforces acquired capability (habituation to fear of death and pain), so subsequent attempts trend toward more lethal methods.
  • Organ injury from the attempt itself: acetaminophen-induced fulminant hepatic failure (rising transaminases then INR and encephalopathy), anoxic brain injury and cervical/airway injury after hanging, rhabdomyolysis and compartment syndrome from prolonged down-time (dark urine, markedly elevated CK, rising creatinine), and aspiration pneumonitis in the obtunded overdose patient. All emergencies.
  • Cardiotoxicity in tricyclic overdose: sodium-channel blockade widens QRS and produces a terminal R wave in aVR, progressing to ventricular arrhythmia and seizure. Emergency.

Complications of treatment

  • Antidepressant-associated activation: the FDA boxed warning applies to patients under age 25; psychomotor retardation may lift before mood does ("energy before hope"), transiently enabling action on pre-existing ideation. Signals are new agitation, akathisia, and insomnia in the first weeks.
  • Antidepressant-induced mania or mixed state in undiagnosed bipolar disorder — mixed states carry the highest suicide risk of any mood state.
  • Serotonin syndrome with overdose or combination therapy: clonus, hyperreflexia, hyperthermia, autonomic instability. Emergency.
  • Lithium toxicity: narrow therapeutic index; tremor, ataxia, confusion, and seizure, precipitated by dehydration, NSAIDs, thiazides, or ACE inhibitors. Emergency at high levels.
  • Clozapine: agranulocytosis (ANC monitoring under REMS), myocarditis in the first weeks (fever, chest pain, eosinophilia, troponin rise), seizures, and severe constipation/ileus. Emergencies.
  • ECT: transient retrograde/anterograde amnesia and post-ictal confusion; esketamine: dissociation and blood-pressure elevation requiring supervised monitoring.
  • Iatrogenic harm from coercion: involuntary hospitalization can rupture the therapeutic alliance and deter future disclosure — an argument for the least restrictive effective setting.

  • Prior suicide attempt is the strongest single predictor of completed suicide; hopelessness predicts eventual suicide better than depression severity score does. Examiners test hopelessness over sadness.
  • Asking directly about suicide does not plant the idea — this is the most commonly tested distractor. The single best next step when a patient hints at wanting to die is always to ask explicitly about ideation, plan, intent, and access to means.
  • The "gender paradox": women attempt more often, men complete 3–4× more often, driven by method lethality (firearms, hanging). A firearm in the home is the highest-yield modifiable risk factor — ask about it and counsel on removal or locked storage.
  • Two agents with disease-specific anti-suicide evidence: lithium (bipolar disorder, recurrent depression) and clozapine (FDA-approved to reduce recurrent suicidal behavior in schizophrenia/schizoaffective disorder). SSRIs treat the underlying illness but are not the "anti-suicide drug" answer.
  • Two highest-risk windows: the first weeks after starting an antidepressant (psychomotor activation returns before mood improves) and the first weeks after psychiatric discharge. A stem describing a depressed patient who "suddenly seems calm and resolved" or is newly energized is describing danger, not recovery.
  • Safety planning, not a "no-harm contract." The Stanley-Brown Safety Planning Intervention plus lethal means restriction is the correct answer; contracts for safety have no evidence and are the classic wrong choice.
  • Confidentiality yields to safety: a patient with active ideation, plan, and intent who refuses admission is held involuntarily under state civil-commitment law. Capacity to refuse other medical care does not override dangerousness criteria.
  • Common distractor to reject: using the SAD PERSONS mnemonic score to decide disposition. Board answers favor structured instruments such as the C-SSRS or ASQ combined with clinical risk formulation, per the APA guideline and Joint Commission NPSG 15.01.01.
